    題名: A composite of exhaled eicosanoids is associated with childhood asthma and its convalescence

    摘要: RATIONALE: Aberrant generation of eicosanoids is associated with asthma, but the evidence remains incomplete and its utility as biomarkers is unclear. Major eicosanoids in exhaled breath condensates (EBCs) of children with asthma and of those during exacerbation and after treatment were assessed. METHODS: A battery of 14 exhaled eicosanoid species was measured using a combination of LC-MS/MS and ELISA in asthmatic children (N 5 686), 81 of who were evaluated at exacerbation and after treatment, and 181 healthy subjects. RESULTS: In the discovery phase, 5 exhaled eicosanoids showed significant difference between asthmatics (N 5 60) and controls (N 5 20). In an expanded case-control study, the levels of exhaled LTB 4 (p<.001), LTE 4 (p<.001), PGE 2 (p<.001) and LXA 4 (p<.001) were significantly elevated, while 15-HETE (p<.001) was decreased, in asthmatic (N 5 626) versus healthy subjects (N 5 161). Receiver Operating Characteristic (ROC) analyses revealed the level of exhaled LTB 4 to be the most discriminative among all parameters, including FeNO. Logistic regression analyses identified a composite of exhaled LTB 4 , LXA 4 and FeNO, together with FEV 1 , distinguishing asthma (sensitivity, 87.5%; specificity, 85.5%). Analysis of 81 asthmatics during exacerbation and convalescence showed significant improvement in lung function (FEV 1 ; p<.001), concomitant with significant reduction in the level of TXB 2 (p<.05) after therapy, while significantly decreased levels of exhaled LTB 4 and LXA 4 were noted in those who had the respective levels at the top 30 percentile during exacerbation. CONCLUSIONS: A composite of exhaled eicosanoids was found to best discriminate childhood asthma, while decreased levels of exhaled TXB 2 and, to some extent, of LTB 4 and LXA 4 , were prominent after treatment.
