    題名: Response to "How can the term SIDS be used without the application of standard definitions?''

    摘要: In this article, we define SIDS by ICD-9 code 798.0, which accounted for 68% of cases, and the codes E911, E912, and E913 as suffocation, accounted for 32% of cases, which should be classified under SIDS. In a previous study (Knobel and Chen, 1996), the authors described the combined average rate by this way to be similar to the death rate in SIDS that is reported from Western countries. We fully agreed with the necessity of the SIDS diagnosis to meet the standard criteria. However, as we mentioned in the article, most infants who died in Taiwan did not undergo autopsies due to cultural reasons. Despite that, the final diagnosis would be made only after a judicial investigation by law. If the infant was reported to die at home, the authorities have to carry out an adequate investigation into the incident, ruling out the possibility of other etiology regarding sudden death like being violently hit by an external force. And local public physicians will be involved in the process so that possible medical conditions could be taken into consideration. If the infant was taken to hospital as OCHA (Out-of-hospital cardiac arrest), the final diagnosis would only be made after reporting to a prosecutor. We believed that the complete judicial system may provide even more reliable SIDS diagnosis.
